Orders from investigators within Tufts should be accompanied by an Interdepartmental Requisition Form.

Orders from investigators at NEMC should have their cost center number, department, and box number filled in on the form.

Orders from all others should have a purchase order number; Visa, MasterCard, or American Express number; as well as a mailing and billing address.
